Title:Vpliv probiotikov na črevesno funkcijo pri otrocih in mladostnikih s celiakijo

Abstract:Uvod: Celiakija je kronična avtoimunska bolezen tankega črevesa, ki nastane kot posledica imunskega odziva na gluten. Pri otrocih in mladostnikih se kaže z različnimi prebavnimi težavami. V zadnjih letih se vse več pozornosti namenja uporabi probiotikov, ki lahko z vplivom na črevesno mikrobioto prispevajo k izboljšanju prebave. Namen diplomskega dela je bil preučiti vpliv probiotikov pri otrocih in mladostnikih s celiakijo. Metode: V diplomskem delu smo izvedli pregled, analizo in sintezo znanstvene ter strokovne literature o vplivu probiotikov na črevesno funkcijo pri pediatrični populaciji s celiakijo. Uporabili smo opisno metodo dela in sistematični pregled literature. Iskanje virov je potekalo v podatkovnih bazah PubMed, CINAHL in Cochrane Library. Pri izboru in analizi literature smo sledili smernicam PRISMA, podatke pa obdelali z narativno sintezo. Rezultati: V končno analizo smo vključili devet raziskav. Ugotovili smo, da dodajanje probiotikov k brezglutenski prehrani večinoma ugodno vpliva na sestavo črevesne mikrobiote, predvsem s povečanjem deleža koristnih bakterij. Vpliv na klinične simptome in črevesno funkcijo pa ni bil enoten. Razprava in zaključek: Probiotiki pri otrocih in mladostnikih s celiakijo kažejo pozitiven vpliv predvsem na črevesno mikrobioto in nekatere vnetne markerje, zlasti TNF-α. Klinični učinki ostajajo manj izraziti, zato so potrebne nadaljnje raziskave z večjimi vzorci in standardiziranimi metodami.

Title:The effect of probiotics on intestinal function in children and adolescents with celiac disease
Abstract:Introduction: Celiac disease is a chronic autoimmune disorder of the small intestine caused by an immune response to gluten. In children and adolescents, it may present with various gastrointestinal symptoms. In recent years, increasing attention has been given to probiotics, which may improve digestion through their effects on the gut microbiota. The aim of this thesis was to examine the effects of probiotics in children and adolescents with celiac disease. Methods: A review, analysis, and synthesis of scientific and professional literature on the effects of probiotics on intestinal function in the pediatric population with celiac disease were conducted. A descriptive method and a systematic literature review were used. Literature was searched in the PubMed, CINAHL, and Cochrane Library databases. The selection and analysis of studies followed the PRISMA guidelines, and the data were processed using narrative synthesis. Results: Nine studies were included in the final analysis. The findings showed that adding probiotics to a gluten-free diet generally had a beneficial effect on gut microbiota composition, particularly by increasing the proportion of beneficial bacteria. However, the effects on clinical symptoms and intestinal function were inconsistent. Discussion and conclusion: Probiotics in children and adolescents with celiac disease appear to have positive effects mainly on gut microbiota and certain inflammatory markers, particularly TNF-α. Clinical benefits remain less evident, indicating the need for further studies with larger samples and standardized methods.
