Title:Strategije preprečevanja okužb sečil, povezanih z zdravstvom, pri hospitaliziranih pacientih z urinskim katetrom

Abstract:Uvod: Okužbe sečil, povezane z zdravstveno oskrbo, pri hospitaliziranih pacientih z urinskim katetrom predstavljajo pomemben klinični in javnozdravstveni problem. Namen zaključnega dela je bil raziskati strategije preprečevanja teh okužb v bolnišničnem okolju. Metode: Izveden je bil pregled znanstvene literature v podatkovnih bazah PubMed, CINAHL in Cochrane Library. Pri iskanju je bil uporabljen PCC-pristop ter vnaprej določen iskalni niz z uporabo Boolovih operatorjev. Vključene so bile randomizirane kontrolirane raziskave, sistematični pregledi in metaanalize v angleškem jeziku, objavljene med letoma 2015 in 2025. Izvedene so bile deskriptivna metoda dela, vsebinska analiza in sinteza. Rezultati: Pregled literature kaže, da so najučinkovitejše strategije preprečevanja okužb sečil povezane z omejevanjem uporabe urinskih katetrov, njihovim čim krajšim trajanjem ter doslednim izvajanjem aseptične tehnike in higiene rok. Pomembno vlogo ima tudi racionalna uporaba antibiotikov ter redno ocenjevanje potrebe po kateterizaciji. Oblikovali smo 4 podkategorije: (1) dejavniki povezani z higieno vstavitve in vzdrževanjem urinskega katetra, (2) dejavniki povezani z usposabljanjem zdravstvenih delavcev, (3) dejavniki povezani z organizacijo in (4) dejavniki povezani z pacientom. Razprava in zaključek: Ugotovitve poudarjajo, da je preprečevanje okužb CAUTI najbolj uspešno ob kombinaciji kliničnih in organizacijskih ukrepov, vključno s standardiziranimi protokoli, izobraževanjem zdravstvenih delavcev in sistemsko podporo.

Med vključitvene kriterije smo uvrstili randomizirane kontrolirane klinične raziskave, sistematične preglede randomiziranih kontroliranih kliničnih raziskav in metaanalize, posamezne kvantitativne raziskave ter sistematične preglede kvantitativnih raziskav, objavljene le v angleškem jeziku, ki obravnavajo strategije preprečevanja bolnišničnih okužb sečil pri hospitaliziranih pacientih z urinskim katetrom.

Title:Strategies for preventing healthcar,-associated urinary tract infections in hospitalized patients with urinary catheters
Abstract:Introduction: Healthcare-associated urinary tract infections in hospitalized patients with urinary catheters represent a significant clinical and public health problem. The aim of this thesis was to investigate strategies for preventing these infections in a hospital setting. Methods: A review of the scientific literature was conducted in the PubMed, CINAHL, and Cochrane Library databases. The search utilized a PCC approach and a predefined search string using Boolean operators. Included were randomized controlled trials, systematic reviews, and meta-analyses in English published between 2015 and 2025. A descriptive research method was employed, along with content analysis and synthesis. Results: The literature review indicates that the most effective strategies for preventing urinary tract infections are related to limiting the use of urinary catheters, keeping their duration as short as possible, and consistently implementing aseptic techniques and hand hygiene. The rational use of antibiotics and regular assessment of the need for catheterization also play an important role. We identified four subcategories: (1) factors related to the hygiene of urinary catheter insertion and maintenance, (2) factors related to the training of healthcare workers, (3) factors related to organizational aspects, and (4) factors related to the patient. Discussion and conclusion: The findings emphasize that CAUTI prevention is most successful when clinical and organizational measures are combined, including standardized protocols, education of healthcare staff, and systemic support
