Title:DOLOČANJE VITAMINA D (25-OH) V KRVNEM SERUMU Z ELEKTRO-KEMILUMINISCENČNIM KOMPETITIVNIM IMUNSKIM TESTOM

Abstract:Namen raziskave bil je oceniti natančnost in pravilnost določitve koncentracije hidroksilirane oblike vitamina D ((25(OH)D) v serumskih vzorcih in podati čim bolj pravilno oceno merilne negotovosti izmerjene koncentracije 25(OH)D z rutinskim testom na analizatorju Roche v serumu preiskovancev. Natančnost in pravilnost testa smo ocenili v različnih klinično pomembnih koncentracijskih območjih: na meji določljivosti analizatorja (22,5 nmol/L), pri klinično pričakovani vrednosti (75 nmol/L) in pri višjih koncentracijah (120 nmol/L). Uporabljena metoda določanja 25(OH)D je elektro-kemiluminiscenčni kompetitivni vezavni imunski test. Pri tej metodi določamo koncentracijo označene beljakovine, ki veže vitamin D, in ki z neoznačeno vezno beljakovino tekmuje za vezavo na hidroksiliran vitamin D. Rezultati so pokazali, da je razširjena merilna negotovost pri 95 % stopnji zaupanja pri meji določljivosti ±6,2 nmol/L, pri klinično pričakovani vrednosti ±12,6 % in pri višjih koncentracijah ±14,4 %. Te vrednosti so manjše od napake, ki jo dovoljujeta zunanja kontrola kakovosti RfB (±30 %) in zunanji program za oceno vsebnosti vitamina D DEQAS (±25 %).

Title:DETERMINATION OF VITAMIN D (25-OH) IN BLOOD SERUM WITH ELECTROCHEMILUMINESCENCE COMPETITIVE IMMUNOASSAY
Abstract:The purpose of the study was the assessment of precision and trueness of the determination of hydroxylated vitamin D (25(OH)D) concentration in serum samples and to provide the most reliable estimate of measurement uncertainty for the concentration of 25(OH)D measured with the routine assay on a Roche analyzer. The precision and trueness of the test was assessed at different clinically relevant concentration ranges: at the limit of quantitation (22.5 nmol/L), at the clinically relevant concentration (75 nmol/L) and at high concentrations (120 nmol/L). The method for the detection of 25(OH)D is an electrochemiluminiscence competitive binding immunoassay. This method determines the concentration of a labeled vitamin D-binding protein that competes with an unlabeled vitamin D-binding protein for the binding of hydroxylated vitamin D. The results showed that the expanded uncertainty with a 95% confidence interval for the limit of quantification is ±6.20 nmol / L, for the clinically relevant concentration ±12.6 % , and for higher concentrations ±14.40 % . These values are smaller than the error permitted by the external quality control RfB (±30 % ) and the vitamin D external quality assessment scheme DEQAS (±25 % ).
