Title:A comparison of the performance of adult basic life support performance among nursing students in out-of-hospital simulated environment in standard and enhanced simulation
Abstract:Simulations are essential for preparing nursing students for demanding clinical situations. This study compared the performance of initial adult basic life support (BLS) steps by nursing students approaching out-of-hospital cardiac arrest (OHCA) in standard versus enhanced simulations. A quantitative non-experimental observational design with a two-group cross-over sequence was used. BLS performance was evaluated per ERC (2021) guidelines; data were analyzed using IBM SPSS Statistics. 24 nursing students participated in the study. Scene safety was verified by significantly more students in the enhanced simulation (enhanced: 87.5 %, 21/24; standard: 16.7 %, 4/24). Agonal breathing was correctly identified by 20.8 % (5/24). Most remaining BLS steps were more frequently performed correctly in the standard simulation: shaking shoulders (standard: 50.0 %, 12/24; enhanced: 4.2 %, 1/24), verbal assessment (standard: 75.0 %, 18/24; enhanced: 37.5 %, 9/24), and breathing assessment (standard: 58.3 %, 14/24; enhanced: 29.2 %, 7/24). Standard simulation promotes technical BLS proficiency, while enhanced simulation strengthens situational awareness. This highlights the need for balanced education to help nursing students effectively manage cognitive load.
